The Licensed Practical Nurse (LPN) delivers nursing care under the supervision of an RN in accordance with the care plan and physician’s orders.

-Reports to : RN / Director of Nursing

• Provide direct nursing care, including medication administration and wound care.

• Observe and report changes in client condition to the RN or physician.

• Assist with client teaching and supportive care.

• Maintain accurate and timely documentation of all care provided.

• Support client independence and comfort in the home environment.

• Current LPN license in the state of Kansas.

• At least 6 months of clinical experience (home health preferred).

• Reliable transportation and current CPR certification.
